Focused Emergency and Urgent Care
Paediatric development and the sick or injured child, adult medical conditions including mental health, respiratory illness and sepsis, plus childbirth and assisting a clinician.
The third FREC 4 component, and the broadest. Paediatrics comes first — how children differ by developmental stage, and how a sick or injured child is assessed and managed. The adult block covers mental health presentations, respiratory conditions and the recognition of sepsis. The remainder gathers what is left of the specification: the digestive system, the stages of labour and care after childbirth, and the responder’s role in assisting a clinician with interventions.

Study guide
The broadest component in either level, gathering paediatrics, adult medical conditions and a final group of criteria that have nothing in common except being what is left. Treat it as three separate revision sessions rather than one.
Paediatric emergency care
The largest block. Children are not small adults, and every criterion here turns on how they differ.
- Developmental stages change assessment, communication and normal values — questions specify an age for a reason.
- The sick child and the injured child are assessed differently, and recognising the seriously unwell child early is the recurring theme.
- Paediatric resuscitation differs from adult, and the differences are the examinable part.
Adult medical conditions
Mental health, respiratory conditions and sepsis, examined on recognition and escalation.
- Mental health presentations are assessed on how you respond and on capacity and safety, not on diagnosis.
- Respiratory conditions are distinguished from each other by history as much as by presentation.
- Sepsis is examined on recognition and on the urgency of escalation; the bank cites NICE guidance, which applies to England.
Digestive system, childbirth and assisting a clinician
The remaining criteria. Small, distinct, and easy to leave until it is too late.
- The digestive system’s components and function are examined at a level that supports abdominal presentations.
- Childbirth is examined by stage of labour, complications, and care of mother and baby afterwards.
- Assisting a clinician has defined boundaries — this criterion is another scope-of-practice question wearing different clothes.
In the exam Read the age in every paediatric question before anything else; it changes the correct answer more often than any other detail. Note that the NICE-derived content in this component is England-scoped guidance, which the app states rather than implying it applies UK-wide — check what applies where you practise.
