The level 4 trauma component, and not a repeat of the FREC 3 one. It is anchored in applied anatomy, goes deep on the chest, and then covers two incident types a responder meets rarely and cannot afford to improvise around.

Applied respiratory, musculoskeletal and nervous anatomy

The anatomy revisited at the depth this component assumes, which is more than FREC 3 asked for.

• Respiratory components are examined by function and by how each contributes to the breathing cycle, not simply named.
• The musculoskeletal criteria distinguish the skeletal system’s components, the system’s primary functions, and the functions of individual components — three separate things that questions keep apart.
• The central and peripheral nervous system distinction is examined explicitly at this level.

Thoracic trauma, kinetics and the systematic survey

The clinical core of the component: what a chest injury does, and the survey that finds it.

• Thoracic injuries are examined by physiology — what the injury does to breathing and to circulation — rather than by name recognition.
• Kinetics of injury extends the FREC 3 mechanism content: energy transfer, and what it predicts.
• The systematic survey and immobilisation are assessed as a sequence with decision points, including when immobilisation is not indicated.

Sexual assault and major incident response

Two exceptional incident types, folded together because both are managed by protocol rather than improvisation. Sexual assault is a minority of this block’s questions despite being listed first.

• Caring for a patient who has been sexually assaulted is examined on consent, on preserving evidence, and on what you say and do not say — alongside, not instead of, the clinical care.
• Referral and support pathways are part of the criterion; knowing where a patient goes next is assessed.
• Major incident response is examined on declaration, roles and the national triage tools — the Ten Second Triage and the Major Incident Triage Tool are the ones the bank cites.

In the exam The exceptional-incident block is where reading carefully matters most: these questions describe situations where a well-meant action is the wrong one. For major incidents, revise the current NHS England triage tools directly — they are short, public documents and they are periodically revised.
