Unit 7 covers prevention and how a dental nurse delivers it, from the biology of plaque and saliva through to the techniques that actually change a patient’s behaviour.

Plaque, biofilm and saliva

The biological foundation. Prevention only makes sense once the disease process is clear.

• Plaque is a structured biofilm, not simply food debris — which is why it is removed mechanically rather than rinsed away.
• Saliva buffers acid, provides minerals for repair and has antimicrobial action; reduced salivary flow raises caries risk sharply.
• Calculus is mineralised plaque and provides a retentive surface for more plaque.

Fluoride

The largest sub-topic in the unit, covering both delivery routes and what happens with excess.

• Fluoride works mainly topically, by promoting remineralisation and inhibiting demineralisation at the tooth surface.
• Topical routes include toothpaste, varnish, gels and mouthrinses; systemic exposure includes water and supplements.
• Fluorosis results from excessive fluoride during tooth development, which is why supplement advice is age- and exposure-dependent.

Toothbrushing, interdental cleaning and diet

The advice itself, and the reasoning behind it.

• Brushing removes plaque mechanically and delivers fluoride; technique, duration and frequency all matter.
• Interdental cleaning reaches the surfaces a brush cannot, which is where much disease starts.
• It is the frequency of free-sugar intake, not simply the total amount, that drives the number of acid attacks.
• Hidden sugars in drinks and processed foods are a common source of unrecognised risk.

Oral health education and behaviour change

Knowing the advice is not the same as delivering it in a way that changes anything.

• Advice that is tailored, specific and achievable outperforms generic instruction.
• Involving the patient in setting the goal improves the odds of it being followed.
• Reinforcement over several visits works better than a single thorough session.

In the exam Expect questions that test the mechanism, not just the recommendation — why fluoride works topically, why frequency of sugar matters more than quantity, why interdental cleaning is separate from brushing.
